The question is, when are Shellholes placed?

According to this bit in B2.1: " ... placing a shellhole counter ... 
immediately after any Original KIA result ... ", we place the Shellhole (and 
thus remove the Trench) *immediately* after making the Original DR.

I can definitely see that as occurring in the sequence: a) Original DR (= 
2); b) = Original KIA; c) = place Shellhole counter; d) = remove Trench 
counter.  And then what?  Resolve the CH, I guess.

That's what this rule seems to say.

I compared this rule with B24.11 (Rubble) and B25.43 (Flame).  For Rubble, 
we are instructed specifically to check *before* resolving the attack; and 
for Flame, we are instructed to check *after* resolving the attack.  In each 
case, the relationship of the "event" to the results is clearly indicated 
(i.e., as "before" or "after").

The instruction for placing a Shellhole is brief and direct.

It doesn't *seem* right that the TEM is replaced *prior* to resolution.  But 
I can find no other instruction about which TEM to use when a Shellhole 
occurs.

I don't like it, but it's hard to argue against that view.  The language is 
pretty clear: "immediately after any Original KIA result".  And nothing 
about placing Shellholes "before" or "after" resolving the attack.  Well, I 
guess the word "immediately" tells us when to place them.
