Yes.  Except during the RtPh, entering (or leaving) a foxhole is always a
2-step process: enter the foxhole's location, *then* enter the foxhole
itself.  (Reverse when leaving.)  Concealment loss, defensive fire, etc., is
always based on the current step in the process.

No, becoming CX is not in itself a concealment-loss activity.  Generally,
the terrain that the advancing unit enters is the determinate of whether
concealment is lost or not.  Sometimes you must also consider what the unit
must do before it can enter the Location (such as, if it must leave a
foxhole in OG first).
