A12.12 says a) is correct.  However, A12.11 allows new ?-counters to appear 
when the stack splits up.

Yes (...*isn't* doing anything...).

Officially, you cannot set up dummies *under* real units since the real 
units set up first and the dummies set up second; A12.12.

Yes, as long as (during setup) all the real units are under all the dummies.
